一、盖梁工程概况本标段共有墩台帽88个，其中墩帽共计58个，台帽共计26个，台帽碎采用C30碎标号，全线墩帽C30磅54个、C40碎4个.分别为主线跨G204大桥C30盖梁18个，C40盖梁为10#墩过渡墩计2个，XX主线跨陈港河中桥C30盖梁8个，D匝道跨陈港河中桥C30盖梁2个，XX匝道跨G204中桥C30盖梁2个，XX匝道跨主线大桥C30盖梁2个，XX主线跨中港河中桥C30盖梁8个,XX主线跨八一河中桥C30盖梁8个，XX如海运河大桥C40盖梁9#、10#主墩2个，引桥C30盖梁34个本标段计划投入盖梁定型模板7套:其中组合箱梁、与现浇箱梁过渡墩盖梁模板3套；独柱墩盖梁模板1套；三柱墩帽模板1套;空心板盖梁1套;如海运河主墩盖梁模板1套;加工抱箍巾10米3套、（blo3米3套.目前，我部桩基正在进行批量施工，首件墩柱已施工完成，我部D匝道跨陈港河中桥3#台台帽作为首件盖梁施工.

二、首件工程的目的和意义本盖梁首件工程我部将精心组织，通过本个首件盖梁的施工，搜集和掌握以下施工数据

1、合理的机械配置和人员组织及各工序衔接；

2、施工工艺是否能够满足规范及设计要求

4、配备一定数量的泗水车,大风天气时，施工便道、施工区域内随时进行洒水消尘处理，使不出现明显的降尘

1、人员组织该盖梁首件工程由我部桥梁一队负责施工，项目部具体分工如下施工总负责XX（常务副经理），负责总体统筹安排；现场总负责:XX（副经理），前场机械、材料、人员安排；技术总负责XX（总工），及时进行技术指导，提供技术支持；后勤保障xx（副经理），负责材料供应，补给；质量检查:XX（项目副总），负责质量检验，尤其是原始资料的搜集、监督和整理，为下一步现浇施工提供可拱参考的详实的数据质检部XX（部长），主要是现场工序控制安全、环保XX（专职安全员），负责现场安全及文明施工其他管理人员依据岗位安排，各伺其责，确保各施工部位、各工序均有人把关

2、“四通一平”情况本盖梁首件工程位于海安西互通区，我部施工便道已经贯通，便道宽度7m,基础采取石灰土，路面采用渣石或碎石铺设，满足施工过程各种车辆通行在海安西互通区安装400KVA变压器一台，主要负责海安西互通区施工用电考虑施工的连续性，防止因停电而造成停工，故设200KVA发电机组2台备用van comply with nationallawsandregulations；at the 2.44m,只需安装端模及侧模即可，底模为砂浆垫层，保证强度及平整度模板接缝处用双面胶夹紧，并且采用玻璃胶进行缝隙处理竖接缝处手感无高差或明显不平整，经技术人员检查合格后涂上脱模剂，墩台帽底模安装施工完成后，由测量队对模板底板的轴线、底模标高、平面轴线进行检查、复测，合格后开始吊装墩帽钢筋墩台帽钢筋在钢筋加工场进行制作成型，在墩台帽施工现场绑扎，采用吊车整体吊装就位为保证钢筋骨架吊装时不变形，采用四点起吊的方法起吊，钢筋骨架的就位必须符合设计及桥涵施工规范要求钢筋就位后，经现场监理工程师检测、验收合格后，开始安装模板侧板侧板的安装必须保证垂直度、整个模板的线形顺直以及保证墩帽的上口截面尺寸

6、盖梁混凝土浇筑与振捣本首件盖梁混凝土为C30采用我部拌和站自拌混凝土，由碎运输车运送至现场后，用吊斗进行入模浇筑待模板全部安装结束并经各项指标检测合格后，进行碎浇筑，碎浇筑时，为保证墩柱脱模后碎表面光洁无气泡，关键在于硅浇筑时的振捣混凝土振捣采用插入式振捣器振捣，振捣在浇筑点和新浇筑混凝土面上进行，振捣时采用快插慢拔的方法,并使插入后的振动棒在校中停留30秒，后再慢慢拔出在振动棒拔出的同时，带出碎中气泡，待第二层碎浇筑时，对第一层碎进行复振，排出碎中残余气泡因模板密封性较好，碎泌出的水份无法排出，需用海绵将柱顶泌出的水份吸干，保证税始终处于同一状态，使之色泽一致.振动器移动间距不超过振动器作用半径的lo5倍并与侧模保持5,10cm距离、插入下层混凝土5,10cm；每一处振动完毕后，边振动边徐徐提出振动棒；并避免振动棒碰撞模板、钢筋在浇捣过程中安排1,2个模板工观察模板情况，发现问题及时处理碎施工时要按要求做好浇筑记录及试块.

7、盖梁模板支架的撤除及养护盖梁浇筑完毕后，待混凝土强度达到25MPa后，开始拆除侧板，然后待混凝土强度达到设计强度的75%后，开始拆除墩帽底模，底模的拆除方法为先将砂筒降低，拆除底模，再拆除10井槽钢和45a号工字钢，最后拆除抱箍拆模时防止托架和模板碰撞墩柱，以免税表面损伤，本首件台帽只需拆除侧模板模板拆除后,采用薄膜覆盖养护，养护时间不少于7d模板使用后，打油防锈

五、质量、安全、环保措施

一、质量控制措施

1、质量目标“零缺陷，优良率95%以上”1原材料合格率100%.2砂浆、混凝土试件强度合格率100%.

（5）质量监控部:配合监理工程师做好工程质量检查；检查指导班（组）自检、互检、交接检的工作.3）、保证墩台帽外观质量的措施使用的模板保证光滑平整，不能有坑洼；使用合格的脱模剂，以保证顺利脱模、混凝土颜色均匀.模板安装注意接缝精度，以避免模板错牙，引起混凝土表面鼓胀.使用合理的混凝土施工配合比和合格的原材料，严格控制混凝土的搅拌时间，以拌熟混凝土，保证混凝土的坍落度和和易性使用较细的振动棒，振捣过程应掌握振捣的时间与间距，振捣时快插慢拔，，保证均匀振捣，防止过振或漏振浇注过程中注意模板的位置，防止出现跑模现象二）、盖梁施工安全措施

2、施工用电安全防护1）配电线路必须按有关规定架设整齐，架空线应采用绝缘导线，不得采用能够塑胶软线,不得成束架空或沿地明敷设施工场地线路均应采取可靠的防护措施2）配电系统必须采取分线配电，各类配电箱、开关箱的安装和内部设置必须符合有关规定开关电器应标明用途.各类配电箱、开关箱外观应完整、牢固、防雨、防尘，箱体以应外涂安全色标，统一编号停止使用的配电箱应切断电源，箱门上锁

3、盖梁施工高处作业的安全措施1）遇有风力在六级以上（含六级，以下同），大雾天、雷电暴雨天等恶劣气候，不得进行露天高处作业.酒后禁止从事高处作业2）高处作业必须戴好安全帽，系好安全带，穿好防滑鞋（解放鞋）并擦净鞋底泥沙，以防滑倒坠落3）供施工人员上下的走道采用门式梯，不得采用悬梯、竹木梯、钢筋梯等等代替，并且必须设置护栏及防没措施施工人员禁止在模板、脚手架上攀登或随运料的吊斗、吊篮和吊物上下4）作业平台的承重必须满足施工荷载要求，平台上不应集中堆放过重的物料和机具高处作业应按规定挂设安全网（立网或平网），安全网内不许有杂物，破损的安全网应及时予以更换5）作业临边应设有高度大于10m的牢固的防护栏杆，并设间距为35厘米的水平横杆严禁高处作业人员向下抛物6）在有坠落可能的地方作业时，必须把安全带挂在牢固的结构上，安全带应高挂低用，带绳长度不应超过3米.7）高处作业需跨越较大空隙时，应从设有安全网的地方通过，不得在未固定的构件上行走或作业.三）、墩台帽施工环保措施施工过程中要采取各种措施控制施工污染，减少污水、废渣、噪音污染紧密结合本首件工程特点，具体措施如下

1、墩台帽施工过程中的碎垃圾等，应使用封闭的专用车辆进行运输，并废弃至指定地点或者在符合规定要求的场地进行有效利用.

2、各种临时设施和场地如堆料场、加工厂等距居民区不宜小于300nl，而且应设于居民区主要风向的下风处.

3、注意保养机械设备，使施工机械维持最佳工作状态，使噪音维持最低噪声水平当施工工地距居民住宅区距离小于150m时，本首件工程安排在白天施工.。
